The real problem starts after the proof of concept
Many industrial AI initiatives do not fail in model development. They fail in the transition to real operations. During a pilot, many things can still work informally: limited data, short decision paths, a highly motivated project team. But as soon as the system moves into production, quality, maintenance, or workforce processes, that informality breaks down.
At that point, different questions start to matter: Who is accountable? Which risk class applies in the real use case? What evidence is available? Who can approve go-live? What happens after an incident? These are exactly the gaps that slow industrial AI down later.
These seven gaps are the ones we see most often.
1. There is no complete inventory
One plant has an assistant for maintenance reports, Quality uses a vision model, Procurement uses a forecasting tool, and HR uses AI-assisted screening. Each team knows its own part. The company does not know the whole picture.
Without a complete inventory, there is no sound prioritization, no clean risk review, and no central view of production AI. In industrial environments, shadow AI is not a theory issue. It is normal operational behavior.
2. Provider, deployer, and operator are mixed up
Responsibilities are rarely simple in industrial setups. The software vendor provides the base capability, an integrator adapts it, an internal team adds data or workflows, and the plant runs the system in production. If those roles are not documented clearly, it also remains unclear who carries which obligation.
The result is predictable: contractual gaps, coordination problems, and missing accountability exactly when a release decision or an incident demands a clear answer.
3. The risk class is assigned generically instead of contextually
One of the most common mistakes is: "This is just an industrial assistant, so it cannot be high risk." That may be true, but it is not a safe assumption. What matters is not the label of the technology but the concrete context of use.
A system that supports documentation is different from a system that influences workforce prioritization, quality release, or safety-relevant decisions. Companies that classify generically are building on assumptions instead of assessments.
4. Documentation and technical evidence are built too late
Many teams start proper documentation only when Procurement, Legal, or a customer wants proof. Then everything is missing at the same time: intended purpose, test evidence, logging design, human oversight rules, monitoring plan, approval status.
The problem is not just the amount of work. It is the quality. Evidence assembled retrospectively under pressure is rarely strong enough for serious audit or customer review.
5. Human oversight remains a slide, not a process
Almost every project claims to have human control. In practice, it is often unclear what that control actually means:
- Who is allowed to intervene?
- Based on which criteria?
- When must an escalation happen?
- Which decisions must never run fully automatically?
Until those points are embedded in the operational process, human oversight is more of a claim than a functioning control mechanism.
6. Logging, incident flow, and reassessment are missing
During a pilot, teams often rely on: "If something happens, we'll talk." That is not good enough in production. Industrial AI needs traceable logs, defined incident criteria, clear owners, and a mechanism that triggers reassessment after critical events.
Without that structure, problems stay invisible for too long. And even once they are noticed, nobody knows who documents them, who decides next steps, and on what timeline.
7. There is no hard gate between pilot and production
This is probably the biggest pattern: go-live happens because the project technically works and the business team wants speed. What is missing is a binding gate that checks whether minimum conditions are actually met before release.
A defensible production path therefore needs a defined gate moment with owner, reviewer, approver, evidence review, open findings, and a documented final decision. Without that gate, production becomes a hope-based model.
What these gaps cost in practice
At first glance, these gaps look like compliance details. In reality, they cost time, trust, and rollout speed:
- Deployments get delayed because required evidence is missing.
- Customer and procurement questions cannot be answered credibly.
- Security, Legal, and business teams work against each other instead of through one operating model.
- Incident responses become chaotic because there is no escalation logic or clear ownership.
Industrial AI does not fail because of the text of the law. It fails because operations are not governable.
What a defensible path from pilot to production looks like
A functioning production path is less complicated than many teams think. What it needs most is sequence and discipline:
- Register the system.
- Clarify role and deployment context.
- Assess the risk class.
- Build documentation and evidence.
- Define human oversight, logging, and incident workflows.
- Allow go-live only through a documented gate.
- Keep monitoring and reassessment active during operations.
This sequence is what turns industrial AI governance into something that actually works in production environments.
